NOTARY Needed!!
We will need the assistance of a Notary Public to notarize the student's health forms on the uniform check-out dates, July 21st, 22nd, and 24th.
If you are a Notary, or know a Notary who would volunteer their time to help, please contact Keith Patrick at 504-9033.
Band CONTRACT/DUES
and UNIFORM Check-out
Band agreement contracts must be turned in and dues
paid in full
before the student can be fitted for their marching uniform. These dues are essential for our band to be able to function.
Please check the attached calendar for uniform check-out dates. If you have any questions regarding your dues, please contact Keith Patrick at 504-9033.
Also, please remember that
every
student (including guard members) must be accompanied by a parent at uniform check-out to have their health forms signed and notarized before being fitted for their uniform. You will need to know your child's last Tetanus shot and provide any other pertinent medical information we might need to have on file for your child.
¹
Tips for a good uniform fit...wear band shoes or tennis shoes, shorts and a T-shirt!
No flip-flops or long pants!
Returned CHECK Policy
If you haven't already, don't forget to purchase a Kroger gift card from the band! This is an easy fundraiser that requires no selling!
We keep up with your card number and when we get a check from Kroger, 4% of your purchases will be added to your child's band account after the band receives $50.00 per family per year. (We must have $5,000 in "reloads" to receive a check.) We get monthly reports from Kroger and the year will be from August to August. You may get and use as many cards as you want (tell all your family and friends) and they will all go to your child's account. Charge your cards at the check-out or service desk with cash, check, or credit card. The gift cards may also be used at the pharmacy and gas station.
Dorothy Walker (896-1501), will have these available to purchase at the booster meetings.
